Advance Care Planning Webinar Series – Fall 2020

An event every week that begins at 4:00 pm on Tuesday, repeating until October 6, 2020

Establish your goals. Overcome your fears.  

Join us for this webinar series to take a “deep dive” into important aspects of planning for the future and end-of-life. Panelists will include Dr. Lucia Jander and members of Hospicare’s interdisciplinary team as well as experts in financial planning and estates.   

  • What life sustaining treatments would you want in the event of a medical crisis?  
  • How would you like to be remembered after you are gone?  
  • How can your communication, medical, and financial plans support your loved ones in the future?   

This will be a three-part series – participants can register for one, two or all three sessions. Three consecutive Tuesdays.   9/22, 9/29, 10/6, 4-5:30pm

Each session will include a presentation and a live Q&A session. Event is free, open to the public, and will be on Zoom. Registration is required. Details about signing on to Zoom will be provided after registration. Registrants will receive an email with printable hand out and links to the documents before each webinar. Printed copies can be mailed upon request.  

The webinar series is a partnership between Hospicare & Palliative Care Services, the Bem Endowment at Hospicare, and the Ithaca College Gerontology Institute. With additional support from the Community Foundation of Tompkins County , the Funeral Consumer Alliance of the Finger Lakes, the Cancer Resource Center, and the Cortland Free Library.

SESSION DESCRIPTIONS 

  • September 22: Health Care Planning and End-of-Life PlanningIn this introductory session we will highlight the importance and foundations of advance planning as related to health care.   
  • Participants will gain a familiarity with health care documents, including the Health Care Proxy and MOLST forms. Discussion will also cover other aspects of end-of-life planning, including writing your obituary, planning your funeral, and making decisions about organ or body donation.
  • Panelists: Dr. Lucia JanderHospicare Medical Director; Edna BrownHospicare social worker; and Angela Mennitto, a member of Funeral Consumer Alliance of the Finger Lakes  

  

  • September 29:  Estate and Financial Planning: Learn about wills and trusts and how a little work now will benefit you and your loved ones in the future.   
  • Participants will learn more about what to consider when creating a will and how to go about the process (hint: it can be free!). Discover ways to structure your finances to ensure your comfort in retirement and meet your long-term goals, including taking care of your family, distributing heirlooms, and defining and creating your legacy. 
  • Panelists: Marshall McCormick, local financial planner; Mariette Geldenhuys, estate attorney; George Ferrari, CEO of the Community Foundation of Tompkins County 

 

  • October 6: How to make Tough Decisions and Have the ConversationsIn this session, we will discuss how to start the conversation with your families, what to discuss, and how to support loved ones who need to be thinking about these things  
  • Bring your questions and concerns about how to have these conversations with your loved ones.  
  • Conversation will be facilitated by Laura Ward, Hospicare’s Manager of Psychosocial Services and Edna Brown, Hospicare Social Worker. 

***The first two sessions will be a webinar format (participants will not have video on) and will be recorded and available afterwards for viewing. The final session will be a Zoom meeting, participants can join with or without video and the session will not be recorded.
