Advance Care Planning 101

January 18, 2022 @ 5:30 pm - 7:00 pm

Establish your Goals. Overcome your Fears. End-of-life planning conversations are hard, and now is the time to have them. Join us for two webinars with Dr. Lucia Jander and the Hospicare & Palliative Care Services’ interdisciplinary team. Workshops can be taken independently of one another.

January 18, 5:30-7pm: Advance Care Planning 101

March 15, 5:30-7pm: Having the Conversations & Organizing Your Info

End-of-life. Dying. Death. These words make people so uncomfortable that they often soften them. We talk about loved ones who “passed away,” or “lost a battle,” or “went to heaven.” Dying is not something we can – or should soften. Death is a part of life and planning for it can make all the difference to the emotional health of those you leave behind. Hospicare encourages people to confront their own mortality, to think about their wishes, and – we hope – to become a little more comfortable with death as a part of life. This is a good time to open the door to those conversations with loved ones and the virtual workshops will cover:

  • The importance of advance care planning
  • Guidance in filling out Health Care Proxy, MOLST and other important forms
  • How to start the conversation with your families and loved ones

Workshops will include a presentation and a live Q&A session. Registrants will receive an email with printable handout and links to the documents. Workshops are FREE but advance registration is recommended.

For more information or to register email jennifer@crcfl.net.

Dr. Jander Bio: Dr. Lucia Jander joined Hospicare as Medical Director in January 2020. Born in Bratislava and educated in Prague, Dr. Jander came to the United States at the age of 24 for her residency at Boston University. She and her husband later moved to Ithaca, where she juggled parenting four children with practicing medicine at local nursing and assisted living facilities. Many in the community know Dr. Jander through her work at Primary Care Internal Medicine of Ithaca, where she continues to attend to her patients. Dr. Jander believes strongly in the value and philosophy of hospice and palliative care and wishes that all of health care used this interdisciplinary model as it gives patients the best chance of having their social, medical, and spiritual needs met.

Edna Brown, LMSW Bio: Edna is a Hospicare social worker with 2 decades of experience in school social work and parenting education and a life-long interest in end-of-life matters. She was drawn to hospice work years ago, when a friend died in the presence of the hospice social worker. She is honored to help patients and their loved ones achieve the best end of life experience possible.
