If you are ready to learn how hospice services can help you or a loved one, contact our admissions staff at 607-272-0212.
We’ll ask you for information and will need to contact your primary physician or specialist (such as an oncologist or cardiologist) if one is treating you for serious illness. We will need to know:Patient’s name, age, address (home address and where the patient is currently)
- Patient’s name, date of birth, address (home address and where the patient is currently)
- Name and contact information for the primary caregiver or healthcare proxy
- Your primary physician (or specialist) contact information
Our admissions staff will collect information from your doctor about the diagnosis and the medical records. Once we have this information, our staff will contact you to schedule an in-person visit with a member of our team.
