Members of the Hospicare care team are assigned to each patient based on their individual needs and desires.
Hospice Medical Director
Dr. Eric Lessinger is board certified in family practice and geriatrics, and has extensive knowledge in pain management. He works closely with the patient's primary doctor to ensure the best care.
Primary Physician
The patient’s physician remains actively involved in care. They provide a detailed medical history to the hospice team, review and approve the hospice care plan, prescribe medication and oversee their patient’s treatment.
Primary Care Nurse
Hospice primary nurses are registered nurses with an expertise in symptom management and end of life care. They provide case management through regular assessment visits and visits on an "as needed" basis. Many have a special Certification in Hospice and Pallitive Care (CHPN)
Certified Social Worker
Our social workers assist patients and their families with practical details and arrangements, as well as helping to explore complex emotional concerns and discuss issues of meaning.
Certified Home Health Aide
Home health aides provide personal care for the patient and are available to make daily visits if necessary.
Spiritual Care Coordinator
The end of life may raise spiritual questions, whatever one’s belief system. Our chaplain is available to people of all faiths to help them find their own answers, and to connect with clergy from their own faith community if they wish.
Patient Care Volunteers
Patient care volunteers are able to support both patients and family members. They may provide companionship, run errands, help with cooking, read aloud, write letters, offer the caregivers a chance to get out, or help out in many other ways.
Bereavement Services Coordinator
The bereavement services coordinator provides counseling sessions, telephone contacts, and distributes regular informational mailings about grieving to family members of hospice patients. Grief support groups are also available to others in the community who seek support after the death of a loved one.

LPN for the Hospicare Residence - part-time (evening and night shifts)
End-of-life care experience preferred.
Hospicare's 6-bed residence is home-like and staffed 24/7 with an RN/LPN and HHA.
All clinical staff work as part of an interdisciplinary team to deliver the high quality of care that is consistent with our mission.
